Role of social worker - challenges and limits of work

1 Abstract The diploma thesis "The role of a social worker in a psychiatric hospital - challenges and limits of work" deals with the profession of a social worker in a psychiatric hospital. The diploma thesis normally consists of a theoretical and an empirical part. The theoretical part is focused on mental illness, the characteristics of serious mental illness, the profession of a social worker working in a psychiatric hospital, multidisciplinary cooperation, the specifics of working with the mentally ill, the reform of psychiatric care and the organization of inpatient psychiatric care in the Czech Republic. In the empirical part, the qualitative research methodology is described and the data, which are the result of semi-structured interviews with social workers and their clients, are interpreted. At the end of the thesis, the main research question is answered, recommendations for practice and proposals for further research are included. Engaging Peer Consultants in the Mental Health Care in the Context of Psychiatric Care Reform in the Czech Republic

The thesis deals with the issue of the current involvement of peer consultants in the care of mentally ill people in the context of identifying their specific needs. The aim of the thesis is to identify the benefits of employing peer consultants for themselves and for the social and health care system of mental health care. At the same time, the thesis aims to identify possible barriers or challenges that arise in the practice of peer consultant work, as well as in their employment. The author uses the concept of recovery and implementation theory to theoretically and conceptually anchor the thesis. Through the perspective of experts working in the field and peer consultants, the thesis provides a mapping of the benefits of the peer consultant role and an overview of their specific needs. At the same time, the author identifies the existence of significant barriers in the current set-up of the system of peer consultant involvement, which are mainly caused by the legislative non-anchoring of their position. The author concludes with recommendations for the future development of peer consultant involvement.

Za tímto účelem byla provedena Hlavním zjištěním práce jsou rozdíly v přístupu k duševnímu zdraví, kdy se Strategie reformy psychiatrické omezuje pouze na oblast psychiatrie a příjemce péče, zatímco mezinárodním diskurzu se tato problematika řeší ěji jako celospolečenský Na druhou stranu je vidět určitá ideová změna představovaná zejména rozvojem komunitní péče, která byla v České republice dlouhodobě zanedbávaná. duševního zdraví se některých ohledech přibližuje světovým trendům, rozdíl přístupech k duševnímu zdraví je ale stále patrný.
Total Home: Care and Social Life in Psychiatric Institution

The thesis is based on three months of participant observation in a long-term care facility in 2010 and interviews conducted during next three years. It draws from a perspectives of medical anthropology and anthropology of institutions. The focus is on an ethnography of institution for clients with chronic mental disease, dementia and substance abuse. The thesis is mapping institution's components and actors, their social life and relationships in the institution. The analyses is based upon the concept of total institution by Erving Goffman, and draws from thoughts of Josef Pieper, Martin Buber, Emanuel Lévinas and Michel Foucault. A long-term facility is an institution, which shares clients and some of problems with classical examples of total institutions, but recent reforms aimed to minimalize features of total institutions. Still it does not seem to be a vital institution and new aspects of social situation of staff and clients are recognized. Among new problems is non-existence of therapy for the inhabitants, their marginalization through poor financial situation in an institution, where lot of services are paid and little continuity to other forms of care of more community and ambulant character. Development and new trends in the care for people with mental illness Abstract: The aim of my bachelor's thesis is to capture developments and new trends in the care for people with mental illnesses. In the introduction, I briefly specify the concept of mental illness and set the division of mental illnesses. Several selected diagnoses of mental illnesses, which are described here are based on professional literature, practical experience and available health statistics. The following is a description of the development of care for people with mental illnesses. From Ancient Times to the Middle Ages, society explained the existence of mental illnesses in humans as the action of supernatural forces. The care for people with mental illnesses was merely an exception during these historical periods. The situation of people with mental illnesses improved during the period of Enlightenment. With the development of medical science, society came to the conclusion that mental illnesses were caused by brain diseases. Psychiatry became a field of science with the first successful therapy treatments. Treatment began to be concentrated in mental institutions, later in psychiatric hospitals. Mortality in population with mental illness in the Czech Republic 2000-2017

Mortality in population with mental illness in the Czech Republic 2000-2017 Abstract The bachelor's thesis focuses on mortality in a subpopulation with mental illness. Risk factors related to mental illness that influence mortality of this subpopulation are described in the first part of thesis. Behavioural, social and patophysiological factors are included. Furthermore the main causes of death of this subpopulation are described and the mortality for individual causes is compared with mortality in the general population from these causes. Data from publication ÚZIS Psychiatrická péče for the period 2000-2017 of outpatient and inpatient psychiatric care are used in the second part of thesis to calculate indicators of incidence, prevalence, hospitalization and in-hospital lethality by sex, age and groups of diagnosis. In-hospital lethality is compared with mortality indicators in the Czech Republic with method of direct standardization and relative risk of death. Number of deaths where the direct cause of death is mental illness are compared by place of death. Mortality indicators in inpatient psychiatric facilities throughout given period are increased, intensity is decreasing successively and shows differencs by gender, age groups and groups of diagnosis. The creation and enforcement of psychiatric care reform in the Czech republic

The aim of the thesis is to find and describe important moments that led to the enforcement of the psychiatric care reform. This reform was adopted 24 years after the fall of the communism. Until then, all attempts to change the psychiatric care system had failed. This healthcare segment has long been underfinanced and mainly institutional care prevails. Often, aftercare is not ensured, so it is very difficult for a person who is released from care to be involved in normal life. Also, stigmatization of people with mental illness prevails. Here, reform represents a solution to these and many other problems that the current psychiatry has. This research should answer the question why this strategy was successfully enforced and what was behind it. Using the theory of three currents and interviews with important actors, it discusses the various events and circumstances which have been used by the policy promoters to successfully achieve the approval of the reform. It examines the different motives of actors, their function and roles in the creation and promotion of the reform. The causes of enforcement are sought both in the Ministry of Health itself and in the EU or interest organizations outside the traditional institutions. Mental Health Policy in Czech Republic

The aim of this work is to compare the goals and approaches of the Psychiatric Reform Strategy issued by the Ministry of Health of the Czech Republic in 2013 with the the World Health Organization (WHO) strategic documents seen as representing current world trends in mental health care. Comparative case study was carried out, which included, in addition to the Psychiatric Reform Strategy, two mental health action plans published by the WHO. The main findings of my work are differences in approaches to mental health, where the psychiatric reform strategy only limits to the field of psychiatry and the recipients of its care, while the world trends that are reflected in the action plans, addresses this issue more complexly as a societal problem. In some respects, mental health policy in Czech republic is getting closer to the world trends, but the difference in approach to mental health is still noticeable.
